•Consent to Treatment

You voluntarily consent to receive a Headspa treatment and understand it is provided for relaxation, and wellness purposes only.

•Medical Disclaimer

This service is not a substitute for professional medical care, nor is it intended to diagnose, treat, cure, or prevent any medical condition, including eczema, psoriasis, or other skin diseases.

•Non-Medical Provider

Our practitioners are not licensed medical professionals and will not prescribe or administer medications.

•Client Medical Disclosure

You affirm that you have fully disclosed any medical conditions, skin sensitivities, allergies, medications (oral or topical), or pregnancy that could affect your treatment. If required, you have obtained approval from your physician prior

to your appointment.

•Allergic Reactions & Liability

You accept responsibility for informing your provider of any discomfort or

adverse reaction during the session. Except in cases of gross negligence or

willful misconduct, you release Altsthetics LLC and its staff from liability for

any allergic or unexpected reactions

•Limited Scope of Practice

You understand that you are responsible for drying your own hair. Altsthetics LLC is operated by a licensed esthetician(s). In accordance with Illinois state laws & regulations, blow drying and hairstyling services CANNOT be performed/provided by estheticians. If a client chooses to dry their hair, tools such as a blow dryer and brushes will be made available for self-service use.

•Session Adjustment & Termination

You understand the provider may modify or end your treatment at any time to

ensure your safety and comfort.

•Questions & Satisfaction

You confirm you have been given the opportunity to ask questions about your

service and that all your inquiries have been answered to your satisfaction.

•Assumption of Illness Risk & COVID-19

You understand that, despite our best efforts to sanitize, there remains a risk of exposure to contagious illnesses (including COVID-19). By attending, you assume that

risk and agree not to hold us liable should you become ill.

•Privacy & Data Use

Your personal or health information you share is kept strictly confidential and

used only to customize your treatment. We will never sell or share your data

with third parties.

•Photo/Video Release (Optional)

From time to time we may ASk if we can take "before & after" photos or short videos for marketing. We will always ask for your permission before photographing/filming
